Direct answer

Spinal decompression and traction use a specialized table to apply a controlled, gentle pulling force to the spine. PT 1st uses an FDA-cleared Chattanooga decompression table. Table sessions are one part of a physical therapy plan that also includes evaluation, exercise, and education. A 10-visit decompression package is available for $600 (evaluation included as visit 1). Physical Therapy 1st Rehab & Wellness — Montebello: 433 N. 4th St., Unit 208, Montebello, CA 90640. Call (323) 246-4616. Open Monday–Friday, 8:00 AM – 7:30 PM.

What spinal decompression and traction are

Traction is a controlled pulling force applied to the spine. It can be steady or intermittent, and it can be applied manually by a therapist or mechanically using a table. PT 1st uses an FDA-cleared Chattanooga decompression table.

“Spinal decompression” is the name commonly used for table-based traction with adjustable force, angle, and cycling. The goal of the table is to create a gentle stretch and unloading of the spine in a controlled position. The table can be used for both lumbar (low back) and cervical (neck) traction.

The table is intended to apply a pulling force that may support the treatment goals identified during your evaluation. It is not a standalone treatment, and it does not “rehydrate discs,” “reverse herniations,” or “repair” tissue. Table sessions are paired with exercise and education.

Who may benefit (educational)

People sometimes explore decompression or traction therapy as part of a physical therapy plan for:

Whether it fits you depends on your evaluation. It is not right for everyone.

Who may not be a candidate

Your therapist screens for reasons decompression may not be appropriate at your first visit. This is a high-level educational list, not a diagnostic checklist:

  • •Pregnancy
  • •Recent spinal surgery, or spinal hardware/implants
  • •Recent fracture or significant bone weakness, such as advanced osteoporosis
  • •Spinal infection, tumor, or other serious medical conditions
  • •Severe or rapidly worsening symptoms

Seek medical care right away for:

  • •New loss of bladder or bowel control
  • •Numbness in the groin area
  • •Sudden or worsening leg weakness
  • •Severe symptoms after trauma

These need medical attention, not a decompression session.

How the 10-visit decompression package works

$600 for 10 visits

The initial evaluation is included as visit 1 of the 10 visits. Each visit includes table time when appropriate, paired with exercise and education based on your evaluation and stage of care. Sessions run about 15–20 minutes.

Package visits do not expire. The package is non-refundable. Each package is for one person.

The decompression package is self-pay (out of pocket) and is not billed to insurance. Call (323) 246-4616 with questions. View insurance & payment options.

Price as of October 2026. Call to confirm current pricing.

What usually happens across visits 1–10

This is a typical progression that is adjusted for each person. There is no promised timeline.

1

Visit 1 — Evaluation and screening

The evaluation is visit 1 of the package. History, goals, movement and neurological screen, a check for reasons decompression would not be appropriate, and a plain-language plan. Table time only if appropriate.

2–4

Visits 2–4 — Early sessions

Conservative settings. The therapist watches how you respond during and after each session. Positioning and comfort are adjusted.

5–7

Visits 5–7 — Building tolerance

Settings may be progressed based on response. Table time is paired with targeted exercise and movement education.

8–9

Visits 8–9 — Strengthening and home strategies

Exercise takes a bigger role. Home program and daily-activity strategies are reinforced.

10

Visit 10 — Re-check

Re-assessment against your starting point and goals. Discussion of next steps, which might be a home program, continued PT, or a referral back to your physician if appropriate.

Some people progress differently, and your therapist may change the plan, pause table sessions, or recommend a different approach based on how you respond.

Why exercise is part of the plan

Table sessions are paired with therapeutic exercise and education because building strength and movement control supports what you do outside the clinic. Exercise is selected based on your evaluation and progressed as your care continues.

A decompression plan at PT 1st includes movement-based care, not just table time. Your therapist will discuss which exercises may support your functional goals and recovery.

Frequently asked questions

What is spinal decompression therapy?
Spinal decompression therapy is table-based traction with a controlled pulling force, used as part of a physical therapy plan. PT 1st uses an FDA-cleared Chattanooga decompression table. The table applies adjustable force, angle, and cycling to create a gentle stretch and unloading of the spine in a controlled position. It is combined with evaluation, exercise, and education.
What is the difference between spinal decompression and traction?
Spinal decompression and traction both use a controlled pulling force on the spine. "Spinal decompression" usually refers to table-based traction with adjustable force, angle, and cycling. The terms are often used loosely in marketing. At PT 1st, table sessions are one part of a physical therapy plan that also includes evaluation, exercise, and education. For a detailed comparison, see our article on spinal decompression vs traction.
How much does spinal decompression cost at PT 1st?
$600 for a 10-visit package. The initial evaluation is included as visit 1 of the 10 visits. Package visits do not expire. The package is non-refundable.
What is included in the $600 spinal decompression package?
The package is $600 for 10 visits at our Montebello clinic. The initial evaluation counts as visit 1. Visits don’t expire, the package is non-refundable, and it’s self-pay (not billed to insurance). Table time is used when appropriate and is paired with exercise and education. Call (323) 246-4616 with questions.
How long is a spinal decompression session?
Decompression sessions run about 15 to 20 minutes. Table time is one part of your visit at our Montebello clinic and is paired with exercise and education based on your evaluation. Call (323) 246-4616 with questions.
What should I wear to spinal decompression?
Wear a gym outfit: comfortable workout clothes you can move in. Your visit includes exercise as well as table time, so clothing that lets you move freely helps.
Can I share the decompression package with someone else?
No. Each 10-visit package is for one person only and can’t be shared or split between people. Package visits don’t expire, and the package is non-refundable.
Can I combine the decompression package with physical therapy billed to insurance?
Yes, if your insurance covers physical therapy. The $600 decompression package itself is self-pay and is not billed to insurance. Separately, regular physical therapy may be billed to your insurance if your plan covers it, and the two can be combined. Coverage depends on your plan, so call (323) 246-4616 to verify your benefits. See our insurance and payment options.
How many sessions of spinal decompression will I need?
It varies. The package is 10 visits with a re-check at visit 10. There is no promised number of sessions that works for everyone. Your therapist will discuss what may be appropriate based on how you respond during care.
Does spinal decompression hurt?
Spinal decompression is intended to feel like a gentle stretch. Settings are adjusted for comfort, and your therapist will ask about what you feel during each session. Tell your therapist if anything feels wrong or uncomfortable. There are no guarantees about how it will feel for any individual.
Is spinal decompression covered by insurance or Medicare?
The decompression package is self-pay (out of pocket) and is not billed to insurance. Call (323) 246-4616 with questions. For general information about insurance and payment options, see our insurance page. If your plan covers physical therapy, regular PT billed to insurance can be combined with the package; see the FAQ on combining the package with insurance-billed PT below.
Who should not have spinal decompression?
Your therapist screens for reasons decompression may not be appropriate at your first visit. These may include pregnancy; recent spinal surgery or spinal hardware; recent fracture or significant bone weakness such as advanced osteoporosis; spinal infection, tumor, or other serious medical conditions; or severe or rapidly worsening symptoms. Screening is done in person by your therapist, and this list is not exhaustive.
Do I need a referral?
The decompression package is self-pay. Referral requirements typically apply when billing insurance, but your physician may have recommendations about whether decompression therapy is appropriate for you. Discuss it with your physician if you have questions.
